Seen that before also a friend of my dad's demonstrated this one time using a hook and the flap skin between your thumb and pointer finger, less hook cleanly out more like a bit of of flesh came with it

Some folk are Martyrs for their sport, he could just as easy have used a bit of pork skin and fat for demonstration purposes.

Had a fly taken out of my thumb last summer by a river keeper using that technique - it was either that or hang around in A&E missing the rest of the day's fishing. The pain was a bit sharp but very short lived. Curiously enough I now debarb all my hooks
