As far as how far east, I assume you're talking ocean-run fish. Steelhead were introduced into the upper and lower Great Lakes starting a century ago and they have now become self-sustaining in many systems along the Great Lakes. Minnesota, Wisconsin, Michigan, Ohio, Pennsylvania, and New York as well as the Canadian GL provinces all have steelhead runs.
As far as the west, I know there are steelhead in the Salmon River and various tributaries in Idaho. I believe that Idaho is the limit as far as eastern progress for sea run fish.
Swinging flies certainly is a tactic the works for steelhead, but I think more than any other fish, success with steelhead, to me at least, depends on tactical flexibility. It's fun to fish with a style I enjoy, but ultimately I'm out there to catch fish, so basically what I'm saying is learn to learn and learn to love nymphing if you want to catch as opposed to fish.
